We are excited to announce that Urban Balance in Chicago and Evanston, IL and In-Home Counseling Services in Antioch, IL has officially merged with KP Counseling. We look forward to continuing providing you care as KP Counseling.  Please use the information below to learn more about the merge and the changes you should expect from the merge:

You will continue with your current clinician through this transition, and we anticipate minimal impact to you as a client. Be advised that new registration paperwork will need to be completed for KP Counseling, and effective July 30, 2024, future billing notifications will come from KP Counseling.

Refresh Mental Health, Inc. will serve as the custodian of your medical record. You may request a copy of your record or transfer your record to another provider by completing the appropriate Release of Information form below and returning via mail or fax:
